Current Concepts of Diuretics


Abstract
Anesthesiologists have developed in their work an intimate relationship with the heart and the lung. By contrast, many anesthesiologist still believe that we stand on uncertain ground with the kidney. This uncertainty may be related to limited access to the kidneys for routine monitoring.
In daily anesthetic practice we can not rapidly evaluate renal perfusion or glomerular filtration, the two most important determinants of kidney function. Instead, we have to rely on indirect manifestations of renal function, and on the monitoring of urinary output.
At times, simply the maintenance of an adequate urine output may be important, but the induction of a negative salt and water balance is by far the more common indication for the use of diuretics.
Diuretics are among the most commonly prescribed drugs. Anesthesiologist must understand their pharmacology, the diseases for which they prescribed, and their appropriate use in the operating room and intensive care unit. This review will discuss the pharmacology of various diuretics with respect to their sites and mechanisms of action, their clinical applications, and common side effects.
